Subject: Reminder to Report Verification Results for Verification Groups V4 and V5 to FAA Access to CPS online
In an August 19, 2016 Electronic Announcement, we provided a summary of guidance on an institution’s responsibility to report the verification results for Verification Tracking Groups V4 and V5 to FAA Access to CPS online, information on using a flat file to submit those reports, and information on reporting deadlines. This announcement provides a reminder of that guidance as well as information on a change to the reporting values.
Beginning with the 2014-2015 FAFSA processing year institutions were, and still are, required to report the verification results of identify and high school completion status using the FAA Access to CPS Online website (https://faaaccess.ed.gov). Reporting should occur no more than 60 days following the institution's first request to the student to submit the required V4 or V5 identity and high school completion documentation. This reporting is in addition to normal required verification reporting to the Common Origination and Disbursement (COD) System. Institutions must report the results (as described below) for any student for whom they received an ISIR with a Verification Tracking Group of V4 or V5, and for whom they requested verification documentation. Institutions should only use this reporting function for students where the CPS placed the student in Verification Tracking Groups V4 or V5. It should not be used in instances where the school has selected the student for verification of identify or high school completion status. In addition, institutions must update a previously reported status for any student whose status changes.
Conditions for Each Identity Verification Results Value
In an effort to improve reporting and data analysis of reporting results, beginning with the 2017-2018 FAFSA processing year, the Department has added a “Value 6” to replace the overlap in Value 3 for students where issues were found with both identity and high school completion status. Previously, institutions were instructed to use Value 3, “Verification was attempted, and no issues were found with high school completion, but issues were found with identity” when there was an issue with both identity and high school completion status.
The following describes the circumstances that apply for each of the V4 and V5 verification results values that institutions must use when reporting using FAA Access to CPS Online.

|Value = 3||Verification was attempted, and no issues were found with high school completion, but issues were found with identity.||The applicant presented (not necessarily in person) acceptable documentation of high school completion status. No documentation of identity was submitted or the identity documentation that was presented was not acceptable and no signed or notarized Statement of Educational Purpose was received.|
|Value = 4||Verification was attempted, and no issues were found with identity, but issues were found with high school completion status.||The applicant presented (either in person or with an original notarized statement) a signed Statement of Educational Purpose with acceptable government-issued photo identification. Either no documentation of high school completion status was presented or the documentation that was presented was not acceptable.|
