If you have registered and cannot attend the conference, you must submit this form as notice of your cancellation, and to process any fee refunds, minus the appropriate cancellation fees. If you did not cancel by close of business (5:00 pm EDT) May 31, 2019, you will be responsible for the full amount of the registration fees, and will be billed.
If someone is attending the conference in your place, the new attendee must submit this form; scroll down this page for the Change of Attendee fields. Once the replacement is processed, a new badge will be created, and no cancellation fees will apply.
